We accept referrals from physicians, hospitals, skilled nursing facilities and case managers across greater Los Angeles.
What to send
A referral moves fastest with the patient's demographics and insurance, the orders for skilled services, a recent history and physical or discharge summary, a current medication list, and the face-to-face encounter documentation. Send what you have. Our intake team will follow up for the rest.
What you can expect from us
We confirm receipt, verify benefits, and schedule the RN start-of-care visit. You'll receive the plan of care for signature and updates whenever the patient's condition changes. We serve patients across the San Fernando Valley, Thousand Oaks and the Conejo Valley, Santa Clarita and Valencia, and the Westside from Brentwood to Bel Air.
