Based in Canoga Park, serving greater Los Angeles
The first weeks after a hospital stay are when things are most likely to go wrong. Discharge papers are long, new medications get mixed up with old ones, and small changes like a few pounds of fluid or a red incision are easy to miss.
Our registered nurses visit at home on a schedule your doctor approves. We check how the body is healing, make sure the discharge plan is being followed, and call the physician early when something looks off. The goal is simple. Stay home, get stronger, and avoid a trip back to the ER.
Our nurses provide post-hospital care in homes across the San Fernando Valley, Thousand Oaks, Santa Clarita and Valencia, and the Westside from Brentwood to Bel Air.

What's included
A full RN visit covering history, vital signs, home setup and the discharge instructions, line by line.
Surgical sites are inspected and measured, with dressing changes as ordered.
We compare the hospital list against every bottle in the home and flag gaps to the doctor.
Patients and families learn which symptoms mean a call to us and which mean 911.
Who it helps
Patients coming home after heart surgery, joint replacement, abdominal surgery, pneumonia, a heart failure admission or a stroke. Hospital discharge planners and case managers can refer directly.
All skilled services require a physician's order. If you don't have one yet, contact our office and we can reach out to the doctor for you. See how Medicare and insurance coverage works.
FAQ
We work from the discharge date and the doctor's orders, and we try to schedule the start-of-care visit as soon after the patient gets home as possible. Contact our office before discharge if you can.
For Medicare home health coverage, yes. Homebound means leaving home takes considerable effort, and absences are short and infrequent. Many people recovering from surgery qualify for a period of time.
Your physician signs the plan of care. Our RN recommends a visit schedule based on the assessment, and it is adjusted as recovery moves along.
