Medication problems are one of the most common reasons older adults end up back in the hospital. New prescriptions get added, old ones never get stopped, and two doctors sometimes order the same drug under different names.
A Violet nurse sits down at the kitchen table and goes through everything, from prescriptions to supplements. We compare what's in the home with what the doctors ordered, set up a system the patient can actually follow, and report anything that needs a prescriber's attention.
Our nurses provide medication management in homes across the San Fernando Valley, Thousand Oaks, Santa Clarita and Valencia, and the Westside from Brentwood to Bel Air.
