Chronic Disease Management from licensed nurses

Living with a chronic condition means making a hundred small decisions a week about food, activity, medications and symptoms. A nurse at home helps those decisions go the right way.

We track the numbers that matter for each condition, such as daily weights, oxygen levels and blood sugar. We teach what the numbers mean, and we contact the physician when a trend points toward trouble.

Our nurses provide chronic disease management in homes across the San Fernando Valley, Thousand Oaks, Santa Clarita and Valencia, and the Westside from Brentwood to Bel Air.

Who it helps

Who this service is for

Patients whose condition recently worsened, who were just diagnosed, or whose doctor wants closer monitoring at home.

FAQ

Common questions about chronic disease management

How is this different from a doctor's appointment?

We see the patient where they live, which shows us things an office visit can't, like what's in the fridge or how hard the stairs are. Findings go back to the doctor.

Can you help with a new insulin prescription?

Yes. Insulin teaching is a common reason for skilled nursing visits.

When should we call 911 instead of the nurse?

Severe shortness of breath, chest pain, fainting, confusion that comes on suddenly, or signs of stroke. Call 911 first.
